Locate a medical provider who is registered to recommend medical cannabis<\/strong><\/p><\/blockquote>\n\n\n\n<p>Ask your current provider if they are a&nbsp;<a href=\"http:\/\/medicalcannabis.utah.gov\/providers\/become-a-qualified-medical-provider\/\">qualified medical provider (QMP)<\/a>&nbsp;registered with the Utah Department of Health and if your qualifying condition may benefit from medical cannabis treatment. If your qualifying condition may benefit from medical cannabis treatment but your provider is not a QMP, your provider may register as a QMP or they may refer you to another provider who is a QMP. If your current provider cannot assist, please refer to our Locate a Provider page or visit our&nbsp;<a href=\"http:\/\/medicalcannabis.utah.gov\/resources\/patient-faqs\/\">Patient FAQ.<\/a><\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>2. Meet with a QMP<\/strong><\/p><\/blockquote>\n\n\n\n<p>To qualify for a medical cannabis card, a patient must meet in-person with a QMP. During your visit, a QMP must complete a thorough assessment of the patient\u2019s condition and medical history and document it in the patient\u2019s record.&nbsp;The in-person visit between a medical provider and a patient could happen before or after the medical provider becomes registered as a QMP with the UDOH.&nbsp;<strong>Do not start your application online until immediately before or after your appointment with a QMP.<\/strong><\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>3. Create a Utah ID account (if you do not already have one)<\/strong><\/p><\/blockquote>\n\n\n\n<p>All patients must first create a Utah ID account before starting an application in the electronic verification system (EVS). &nbsp;A Utah ID account can be created by going to&nbsp;<a href=\"https:\/\/id.utah.gov\/\">id.utah.gov<\/a>. For Utah ID account creation instructions,&nbsp;<a href=\"https:\/\/idhelp.utah.gov\/account-creation.html\">click here.<\/a><\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>4. Review the EVS user guide and apply online for a medical cannabis card<\/strong><\/p><\/blockquote>\n\n\n\n<p>Before starting an online application, a patient should review the EVS Patient Application User Guide. After reviewing the guide, the patient may go online in the EVS and begin the medical cannabis card application process.<\/p>\n\n\n\n<p>The EVS Patient Application Guide can be found&nbsp;<a href=\"http:\/\/medicalcannabis.utah.gov\/resources\/evs-help\/\">here.<\/a><\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>5. After a patient creates a profile in the EVS, a patient\u2019s QMP goes online and issues a recommendation<\/strong><\/p><\/blockquote>\n\n\n\n<p>Once a patient has initiated their application online, their QMP must log in online to the EVS and complete their section of the patient\u2019s application, which includes the recommendation for medical cannabis treatment.<\/p>\n\n\n\n<p><strong>NOTE: If you have been diagnosed with a non-qualifying condition, you must upload the&nbsp;<a href=\"http:\/\/medicalcannabis.utah.gov\/resources\/compassionate-use-board\/\">Compassionate Use Board Petition form<\/a>&nbsp;to the EVS.<\/strong><\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>6. Pay medical cannabis card application fee online<\/strong><\/p><\/blockquote>\n\n\n\n<p>The patient returns to their profile in the EVS and pays their medical cannabis card application fee online. At this point, the completed application will be submitted online to the UDOH.<\/p>\n\n\n\n<p>Medical cannabis card fees:<\/p>\n\n\n\n<ul><li>Patient Card (initial): $15<\/li><li>Patient Card (first 90-day renewal): $5<\/li><li>Patient Card (six-month renewal): $15<\/li><\/ul>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>7. UDOH reviews medical cannabis card application&nbsp;<\/strong><\/p><\/blockquote>\n\n\n\n<p>The UDOH will review the patient\u2019s medical cannabis card application and confirm whether all requirements have been met. For adult patients ages 21 and older with a qualifying condition, the application review process will be completed 15 days or less from the date the application is submitted. For minor patients younger than age 21 and adult patients without a qualifying condition, the application review process will be completed 90 days or less from the date the application is submitted, as these applications must be reviewed by the Compassionate Use Board.<\/p>\n\n\n\n<p>Patients younger than age 21 and adult patients without a qualifying condition will receive an email regarding the status of their Compassionate Use Board petition.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>8. Receive medical cannabis card<\/strong><\/p><\/blockquote>\n\n\n\n<p>If all requirements have been met, the UDOH will issue the card and email the patient a copy of their medical cannabis card. The patient can then save the medical cannabis card on their smartphone or print it out.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote\"><p><strong>9. Purchase medical cannabis from a medical cannabis pharmacy<\/strong><\/p><\/blockquote>\n\n\n\n<p>Once a medical cannabis card is issued by the UDOH, a patient is ready to purchase product from a medical cannabis pharmacy.&nbsp;To purchase medical cannabis, a patient must bring their medical cannabis card and a valid form of photo identification, such as a driver\u2019s license, with them to the pharmacy.<\/p>\n\n\n\n<p class=\"has-text-align-center\">Information was obtained from The State Of Utah&#8217;s website  https:\/\/medicalcannabis.utah.gov\/patients\/i-am-a-patient\/service.<\/p>\n\n\n\n<div style=\"height:64px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n<\/div><\/div>\n","protected":false},"excerpt":{"rendered":"<p>Utah Cannabis Card Application Process MEDICAL CANNABIS CARD APPLICATION PROCESS Please review the following outline that describes the card application process: 1.
